Depression, fear, and anxiety are some of the most common and uncomfortable emotions that we can experience at some point in our lives. Through counseling and treatment, we are able to help you recover motivation, perspective, and joy that you once had in your life.
Many individuals can experience symptoms associated with painful and traumatic circumstances. Anxiety, fear, and hopelessness are a few emotions that can linger post traumatic events. We can help you overcome these symptoms and guide you through the process of grief and healing.
Relationship counseling can be beneficial to couples who are looking to strengthen their emotional connection, in all stages of their relationship. Therapy sessions are held with both couples and is a supportive place to discuss issues and solutions to better strengthen your relationship.
We provide diagnostic assessments, individual, group and family therapy along with case management services. We accept only Medicaid and Medicare with no fees paid by patients. See our Strategic Plan at the end of our website for more info on Community Healthcare.
Chuck Larrick, an independently licensed mental health counselor, was providing counseling for residents is a rural Ohio nursing home in the mid 1990’s. Many of the residents were there for short-term rehabilitation and presented with mental and emotional needs that required professional intervention. Upon their discharges, it was easy to arrange for follow-along medical and personal care services in their homes, but finding an agency that offered in-home mental health counseling was nearly impossible.
The nursing home administration viewed the mental health counseling as an extremely important part of each person’s rehabilitation and decided to share the expense with the counselor to continue the counseling for a limited time in some of the discharged resident’s homes.
After a few months it was discovered that the State of Ohio had a Medicaid waiver program, known as PASSPORT that paid for home health care and that this waiver included the provision of mental health counseling. Since there were no home health agencies interested in providing this service, Mr. Larrick applied to be a PASSPORT provider. Once Mr. Larrick began providing the service, referrals kept coming from the Passport case managers and soon reached a number that he could not accommodate. He then formed Community Home Healthcare, LLC and began hiring and contracting with other licensed counselors to help with the ever increasing caseload.
The need is evident as evidenced by the fact the Community Home Healthcare now has a service area that includes fifteen Ohio counties. Community Home Healthcare, LLC continues to meet the need by employing and training licensed professionals who are committed to continuing education and development in home-based counseling to serve individuals whose ability to travel to office-based appointments is significantly challenged.

Introduction
Community Healthcare Associates strategic plan incorporates information from a variety of sources including stakeholders, patients, and family members. The strategic planning is considered an evolving process and will be reviewed and updated on an annual basis.
The document has been organized using the CARF-recommended format, along with the addition of a human resource section. Within each section, there is an analysis of (1) strengths/opportunities and (2) weaknesses/threats and (3) a goals statement and (4) responsibilities.
Community Healthcare Associates strategic plan will be accessible to stakeholders, the general public and patients by being distributed via:
1. Community Healthcare Associates website;
2. Displayed at sites;
4. Available for review by Community Healthcare Associates employees.
Expectations of Persons Served
Strengths/Opportunities
Patient satisfaction surveys are conducted on an annual basis. Survey results for FY 2022 indicate over 90% of respondents rated their service as good or excellent. Community Healthcare Associates Behavioral monitors patient access to services annually.
Weaknesses/Threats
Community Healthcare Associates is striving to serve as many individuals as possible, however; the ever-increasing changes to the Ohio Department of Mental Health’s Medicaid program changes have causes an issue with the overall future sustainability of Medicaid for some patients.
Goals
A major goal will be to better communicate and work with patients and family members to manage the patient’s care. We will know communication efforts have improved by seeing an overall improvement in client outcomes.
Community Healthcare Associates considers the satisfaction of clients, stakeholders and personnel to be at the very important. It shall be a goal that FY 2023 is to develop a client satisfaction survey program that will reflect changes within the service model at Community Healthcare.
Responsibilities:
Community Healthcare Associates employees continuously work on exceeding patient expectations and the company mission. The Management Team will continue to work toward an improved care and programs for all patients. All team members along with the Human Resources Department will continue to recruit and hire quality staff. The Performance Improvement Committee will continue to gage overall patient satisfaction throughout the year and will report on these findings.
Expectations of Other Stakeholders
Strengths/Opportunities
Management and program staff are always eager to understand and develop collaborations with stakeholders, to bridge the gap between expectations and reality related to the development and implementation of services. In other words, we always want to clearly articulate what we can do and what we cannot do and explain why. We plan to continue the practice of surveying stakeholders as a means of gauging awareness of Community Healthcare Associates services.
Weaknesses/Threats
In many areas, stakeholders continue to have high expectations of area service providers. Due to serious budget cutbacks, many area agencies are reducing services.
Goals
To alleviate this situation, Community Healthcare Associates is expanding in service areas and staffing. We plan to meet the areas expectations in terms of scope, availability and accessibility of many programs and services.
This includes the need to recruit additional providers.
Community Healthcare Associates will strive to obtain feedback from a variety of stakeholder sources as our child and adolescent program grows, with an emphasis being placed on the following:

Responsibilities
The Performance Improvement Committee is responsible for surveying stakeholders yearly. The results of the stakeholder survey are discussed. To address staffing constraints, Human Resources will continue to seek highly qualified and effective individuals to fill positions.

Community Healthcare Associates has many strengths which include a broad array of services, the goodwill of the communities we serve, positive name recognition and a core of dedicated and experienced staff.
Community Healthcare Associates has maintained several evidence based or promising practices. Some are continuing to evolve, while others have been a mainstay of Community Healthcare Associates services.
Weaknesses/Threats
Managing clinical services in a unique business model complicates the dissemination of information and the provision of training opportunities.
Goals
Continue the provision of quality services by maintaining fidelity to the evidence-based practices presently in use by Community Healthcare Associates clinical staff; ensure that new staff are trained in the techniques/methods of evidence-based practices in a timely manner after being hired.
Become recognized as a local leader in the effort to improve care and overall health outcomes.

The Management Team Members will be responsible to ensure that these goals are met in the timeframe that has been established. Responsible parties will ensure that funding and training are reviewed often to allow for the continuation of competitive practices.

Community Healthcare Associates management continually scans the environment in search of new sources of revenue. A direct result of these efforts was the establishment of a Community Healthcare Associates acceptance of Medicare/Medicaid services in 2018. The billing service continues to generate revenue and is an asset to patients who without it would not be able to receive the services they need.
Weaknesses/Threats
The expectation from stakeholders and the community providers could provide levels of uncompensated care is a continuing source of tension.
Goals
Community Healthcare Associates will continue to seek ways to enhance revenue by evaluating and modifying or replacing billing software, improving clerical and billing functions, improving authorizations for services to managed care clients, and reduce denial rates.

The Chief Executive Officer in concert with our financial firm to ensure that Community Healthcare Associates remains fiscally sound and that payments are posted efficiently and accurately. Management staff will report any financial opportunity available (programs, etc.) that will aide in the overall mission of Community Healthcare Associates to provide effective care to patients.

Continue to develop collaborations to address the multifaceted needs of persons with emotional and mental illnesses.
Continue to work on access to services and develop strategies to decrease wait time for outpatient counseling and diagnostic assessment service appointments.

All Community Healthcare Associates staff is tasked with continually identifying service needs and reporting those needs to their supervisor. Community Healthcare Associates priority is to expand in service area’s and meet those expectations and needs.
Regulatory Environment

Community Healthcare Associates is fortunate to have experienced staff. This experience has served Community Healthcare Associates well in meeting certification requirements of a number of regulatory bodies including the Ohio Department of Mental Health (OMHAS) and the Commission on the Accreditation of Rehabilitation Facilities (CARF).
Weaknesses/Threats
Our staff members have training needs in terms of (1) individualized position requirements (2) departmental standards, and (3) professional licensure requirements. These multiple demands tend to have a negative effect on staff productivity.

Earn a three-year CARF reaccreditation at the 2022 survey.

HR Department staff are available on a continuous basis to provide basic personnel information, interpret personnel policy and procedure, answer staff questions.
Further, the department handles most recruitment responsibilities, including advertising, screening resumes and applicants. HR staff ensures well maintained employee personnel files and monitor access to these files to ensure accuracy and confidentiality.
Weaknesses/Threats
It is becoming increasingly difficult to recruit credentialed staff, particularly independently licensed therapists. Community Healthcare Associates often is constrained by geography.
Goals
The HR Department will stay abreast of the latest developments in recruitment strategies and monitor compensation and benefit trends within the industry to ensure Community Healthcare Associates remains as competitive as is financially feasible.
A major goal is to offer competitive salary options to employees and to recruit and retain highly motivated, skilled personnel who will make a commitment to Community Healthcare Associates and its reputation in the community.

The IT department is responsible for maintaining multiple systems for Community Healthcare Associates such as the Practice Fusion EHR program and EZclaim.
Weaknesses/Threats
There are always surprises that can happen daily, from a basic PC problem to server failures. Additional threats are external and can be such things as internet-based problems, firewall functionality, PC’s are up to date with their antivirus software, and E-mail is being scanned.

We are a comprehensive behavioral health agency that is currently working toward integrated care. The potential and overall capabilities of Community Healthcare Associates are very strong. Staff are experienced, dedicated to the mission and compassionate with their care. Community Healthcare Associates infrastructure is strong and meets our needs. Community Healthcare Associates enjoys accreditation from CARF and Ohio Department of Mental Health.

Funding continues to be a factor in many areas of service delivery.

Community Healthcare Associates understands the importance of ensuring that the collection of data is brought into the strategic planning process to improve overall business function and service delivery.
Community Healthcare Associates has a very experienced staff that acknowledges the importance of performance analysis and takes an active role in reviewing data, conducting focus reviews on identified trends, and integrating this information into Community Healthcare Associates performance improvement.
Community Healthcare Associates publishes the Strategic Plan on our website for the public to review as well as making copies available at our main office to ensure that clients and stakeholders have access to the plan and its contents.
Weaknesses/Threats
One identified weakness is our Practice Fusion Electronic Health Records. There are functions and reports needed that are not included in the software. Supervision signatures, performance analysis and certain demographics to name a few.
Goals
Community Healthcare Associates will continue evaluating Practice Fusion to determine workarounds within Practice Fusion or possibly replace with more adequate EHR.

Community Healthcare has a good reputation for serving elderly individuals in many area counties.
Community Healthcare Associates has the ability to provide more office-based services to all age groups at our office at 209 Seneca Ave., Byesville, This would be an opportunity to provide more services in the community and would help improve the overall finances of the agency.
Weaknesses/Threats
By over-focusing on home-base services for the elderly, Community Healthcare Associates is providing services that are considerably more expensive than office-based services. Therefore, current Medicaid and Medicare rates are inadequate, and other more comprehensive agencies would have an advantage in competition.
Goals
Community Healthcare Associates will continue to reach out to serve all patients with a variety of resources and services. Our marketing plan is to focus on the youth market for our child and adolescent programs and services.
